You can’t always be there. We can.

Mom’s Home Readiness Plan™ for Aging in Place

Mom wants to stay home. The house needs a plan too. We help families organize the home’s major systems, service history, known concerns, important contacts, and the things that should continue to be watched over time.

Mom wants to stay home.
The house needs a plan too.

Helping Mom or Dad remain at home involves more than responding when something breaks. The house has systems, service providers, maintenance history, warranties, seasonal needs, and small changes that can be difficult for a family to keep track of — especially when adult children cannot regularly be there.

Mom’s Home Readiness Plan™ creates an organized picture of the home so your family knows what is already known, what deserves attention, and what should continue to be watched over time.

What Is Mom’s Home Readiness Plan™?

Mom’s Home Readiness Plan is a practical property record created to help families understand and organize the home as it exists today.

It establishes a starting point for the house: the major systems, known service providers, maintenance history, readily observable concerns, important property information, and items the family may want to continue watching.

It is not a plan for moving Mom out of her home.

It is a plan for helping the family stay better informed while Mom or Dad continues living there.

The Problem Usually Isn’t Finding Someone to Make a Repair

Most families can find a plumber when there is a plumbing problem, an HVAC company when the air conditioning stops working, or a roofer when the roof needs professional attention.

The harder questions often come before that:

  • When was the HVAC system last serviced?
  • Is that water stain new or has it been there for years?
  • Who worked on the roof last time?
  • Is the warranty information somewhere in the house?
  • Was that exterior damage there before the last storm?
  • Has anyone been keeping track of the small concerns that have accumulated?
  • Who in the family actually has the current picture of the home?

A Home Readiness Plan gives the family a place to begin answering those questions.

What Goes Into Mom’s Home Readiness Plan™

Home & Property Baseline

We begin by becoming familiar with the property and establishing a point of reference for the home as it exists today. Relevant photographs and readily observable conditions can become part of that baseline.

Major Systems & Equipment

Important information about major home systems can be documented, including heating and cooling equipment, water heater, generator, pool equipment, roof information, or other significant systems associated with the property.

Service Providers & Maintenance History

Known contractors, service companies, recent maintenance, warranties, and other useful property information can be organized so the family is not starting from scratch every time something comes up.

Known Concerns & Items to Watch

Not every visible change requires immediate action. Some items simply need to be documented so the family can watch for changes and determine whether professional attention becomes appropriate.

Family Contacts & Access Information

The plan can establish the appropriate family contact, communication preferences, property access procedures, and other practical information that becomes important when the family is not physically present.

Not Sure Where to Start?

Mom’s Home Readiness Checklist is a free self-guided resource families can use to look at maintenance, major systems, exterior conditions, property access, documentation, and family visibility with fresh eyes.

The checklist helps identify the questions. The Home Readiness Plan helps organize the answers.

The Initial Home Baseline

For families who want to move beyond the self-guided checklist, the next step is our Initial Home Baseline.

This is where we begin getting to know Mom or Dad’s property and establish the information that becomes the foundation of the Home Readiness Plan and Mom’s Home Record.

Initial Home Baseline: $395 one-time

The baseline is not a professional home inspection. It is an orientation and documentation process designed to establish continuity for the property going forward.

A Plan Is Most Valuable When It Stays Current

A home does not remain frozen in time. Seasons change, storms come through, equipment ages, repairs are completed, new concerns appear, and different professionals may work on the property.

That is where ongoing Home Stewardship comes in.

Our Home Stewardship Membership builds on the Initial Home Baseline through monthly exterior Home Watch visits, quarterly Home Stewardship Reviews, family updates, photo documentation, and continued maintenance of Mom’s Home Record.

Home Watch is part of the service. Home Stewardship is the larger relationship.

When Something Deserves Professional Attention

The purpose of the Home Readiness Plan is not to diagnose building systems or determine what repairs should be made.

When something appears to deserve a closer look, an appropriately qualified professional should evaluate the condition and recommend any necessary work.

When requested, It’s Mom’s Home can help facilitate communication with qualified local service providers. The homeowner or authorized family member remains in control, chooses who to hire, authorizes any work, and contracts directly with the service provider.

Our role is to observe, document, communicate, and facilitate.

Home Readiness — Not a Home Inspection or Personal-Care Assessment

Mom’s Home Readiness Plan™ focuses on the property.

It is not a professional home inspection, engineering evaluation, contractor assessment, medical evaluation, personal-care assessment, or determination of whether someone can safely remain in their home.

It’s Mom’s Home does not diagnose building systems, perform contractor work, supervise repairs, manage construction, or provide medical, nursing, companion, or personal-care services.

Our purpose is to create better visibility into the home, maintain continuity, and help keep the family informed.
